Requirements to become an Alay Kapwa ASSOCIATE Member
( 2 to 17 years old )
STEP 1: Accomplish the Membership Form
Ask a set of the Membership Form at the Membership Section. Accomplish the forms (parents for non-writing children).
STEP 2: Deposit the Initial Share Capital
Deposit the prospective member's share capital at any of the tellers. After depositing the money, the prospective member is already an ASSOCIATE MEMBER.
Requirements to become an Alay Kapwa REGULAR Member
(18 years old and above)
If you are a resident of Nueva Vizcaya:
STEP 1: Attend the Pre-Membership Education Seminar (PMES) You may personally inquire for the next PMES schedule and register for a slot at 2/F Nueva Vizcaya Alay-Kapwa MPCO Bldg., Mabini St., Poblacion South, Solano, Nueva Vizcaya, or you may opt to inquire via call or text by calling telephone number (078) 326-5080 or mobile number 0905-193-1422.
STEP 2: Deposit the Initial Share Capital The prospective member will be asked to deposit a total of P4, 380 (inclusive of P3,800-Share capital; P380-MAAB/Death Aid Benefit; P120-ID; P50-Membership Fee; P30-Accident Benefit Fund) ) or P16,200 (P15,000-Share Capital; P1000-MAAB/Death Aid benefit; P120- ID; P50-Membership Fee; P30-Accident Benefit Fund) in his/her savings account in the Coop.
STEP 3: Credit Investigation (CI) The Coop will conduct a CI to ensure that the information the prospective member has given are accurate. When the result is positive, the Membership Section will contact the prospective member for the schedule of the second seminar.
STEP 4: Borrower's and Ownership Seminar This seminar will explain anything and everything you need to know about the lending services that the Cooperative is offering. Also, the prospective members will be shown around the Cooperative's divisions to show the services and each division has to offer.
The P4, 380 or P16,200 will be withdrawn from the savings account of the member to be transferred to his/her Share Capital. He/she will then become a REGULAR MEMBER.
If you are a resident of Nueva Vizcaya but is currently working abroad:
STEP 1: Download and accomplish the Membership Forms available online Download forms: [ Membership Agreement ][ Beneficiary Listing ][ Personal Data ] Upon download, read and accomplish the Membership and Subscription Agreement and Application Forms. After properly filling it out and signing the downloaded forms, scan them and send to our email address at firstname.lastname@example.org or email@example.com
STEP 2: Interview When the sent application forms are received, a representative from the membership Section will contact you for an interview via Skype, Facebook Video Chat, etc.
STEP 3: Deposit the Initial Share Capital. Deposit the P4,380 Initial Share Capital and Other fees (inclusive of P3,800 Share capital, P380 for the Mutual Aid Assistance Benefit Fund, P120 for ID, P30 for the Accident Benefit Fund and P50 Membership Fee) or the minimum P16,200 Share Capital and Other fees (P15,000 Share Capital, P1000 for the Mutual Aid Assistance Benefit Fund, P120 for ID, P30 for the Accident Benefit Fund and P50 Membership Fee) to any of the bank accounts of the Cooperative and send a notification to the interviewer once payment has been made. In return, upon confirmation that the amount has been received, the prospective member will be updated of the status of his/her membership. He/she will become an Associate Member after the processing.
STEP 4: Attend the seminars When the Associate Member goes back to the Philippines, he/she is obliged to go to the Nueva Vizcaya Alay-Kapwa MPCO Office with the original Membership Forms in order to schedule him/her for the PMES and Borrower's and Ownership Seminar. After the seminars, the Associate Member will undergo Credit Investigation. And if the CI yields a positive result, he/she will become a REGULAR MEMBER.
